Railway Medical Group are looking for a Practice Nurse to join our friendly and dynamic team in Blyth, Northumberland.We are an expanding GP Practice with over 27,300 patients in South East Northumberland, and are dedicated to improving the health and wellbeing of our diverse patient community.We are looking for a motivated team player who can demonstrate a high standard of clinical excellence and knowledge to provide a safe, effective and easily accessible range of health services.Main duties of the jobChronic Disease Management to include: Diabetes, Asthma, COPD, Chronic Heart Disease, Hypertension, to develop and implement care plans for patients with Long term conditions, promoting self-care.About usRailway Medical Group is a key healthcare provider in Blyth, Southeast Northumberland, serving over 27,300 patients across a diverse community.
