Here at Wellspring, we are dedicated to patient-centred care and making a positive difference in our community.The role will include long-term condition management, childhood immunisation, cytology, smears, and travel vaccinations.The successful candidate will also be confident, have excellent organisational skills, and be able to self-manage.Main duties of the jobOur practice nurses run their own clinics independently, with appointment lengths of varying durations dependent on the reason for the appointment.We use SystmOne. We have recently moved to a year-of-care birthday review system for long-term conditions.Our healthcare assistant offers routine screening and supports the team in a range of duties.Our long-term plan for the nursing team is for our nurses to be able to provide all-round care for our patients and to be confident in all areas of the role.About usWe have just over 6400 patients and our list size grows steadily. We have 6 part time doctors not including our GP registrar. We also have our nursing team and a medicines management team including prescribing pharmacist (who supports the nurses in managing long term conditions).The wider team works well together and we have regular team meetings to learn together. responsibilities1 General- To provide general nursing care to patients as appropriate- Take and record full medical notes on all consultations using the computer system- Update and amend the medical computer system with details of ailments, treatments and other advice needed to maintain a full summary- To provide advice, consultation and information about general health and minor ailments and to refer to other members of the practice team as necessary- Perform investigations as necessary, e.g phlebotomy, swabs and other specimens including urine testing as appropriate- Perform ear care following current guidelines and signposting appropriately- Apply and change routine dressings as necessary2 Health Promotion- to provide smoking cessation advice, healthy living, weight management etc- to provide travel advice and administer immunisations as appropriate using current travel immunisation advice guidelines- perform new patient health checks following protocols and template- perform well person checks on registered patients as appropriate- administering infant immunisations3 Long Term Conditions- perform routine asthma/COPD checks on patients following current guidelines- help to manage call and recall programmes for patients in conjunction with other members of the primary health care team- Offer Spirometry checks in conjunction with HCA- Monitor patients with diabetes, hypertension and heart disease following current guidelines- Manage patients within the remit of the COPD programme providing education, care plans and personal follow up after exacerbations.- Monitor QoF performance (and other relevant performance standards) and support the team in following up non-attenders4 Well Woman Care- Offer pill checks following current guidelines- Administer depoprovera injections following current guidelines- Perform cervical smears following current guidelines, complete template and manage follow up system- Perform cervical swabs as appropriate- Offer well woman advice, including family planning guidance- Use recall system to manage call and recall for cervical screening5 Team working- Attend clinical team meetings on a weekly basis- Monitoring and training junior nursing/healthcare assistant staff as appropriate- Supporting other members of the PHCT within your particular areas of expertise especially family planning and respiratory care- Attend in-house staff training sessions (fire, etc) and carry out all necessary mandatory training- Attend Time Out and other appropriate locality events as arranged- Support the monitoring of clinical stock and reordering as necessary6 Personal development- Participate in the appraisal process- Attend training updates as necessary in order to maintain clinical knowledge and
