- Provide clinical assessment and direct the management of patients
- Be autonomous in decision making, including judgments involving highly complex facts/situations that require the analysis, interpretation, and comparison of a range of options in relation to the care/management needs of the patients on a day-to-day basis within the clinical environment
- Identify need for change, leading innovation, and managing change including service development, developing a case for change, and negotiate and influence change
- Carry out complex patient clinical reviews, and feedback complex clinical information relating to physical and mental health to enable decisions in care management to be agreed upon and/or changed which may involve differences of opinion
- Lead and chair ward reviews, including initiating and titrating treatment plans
- Prepare medical reports
- Support doctors' development by arranging opportunities in other clinical areas for them
- Review formal patients and work in collaboration and joint decision making with RCs in respect of ongoing detention and least restrictive principles
- Provide clinical assessment using advanced knowledge and clinical skills, to provide diagnoses both differentiated and undifferentiatedpatients, and develop management plans for optimal care
- Provide Advanced Clinical Practitioner Strategic Service clinical leadership
- Responsible for policy and service development
- Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
- Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
- Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
- 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
- Numerous mental health and learning disabilities services
- Professional registration (NMC, HCPC)
- Level 7 Education (master's degree) relevant to area of clinical practice
- Advanced specialist knowledge of patient pathways
- Advanced theoretical knowledge and practical experience to carry out clinical consultation and/or examination relevant to area of practice as an autonomous clinician
- Independent Prescribing qualification (if required for role)
- Where appropriate Non-Medical Authorisation of Blood Products
- Management/Leadership qualification or possess equivalent skills and experience
- Relevant teaching qualification
- Evidence of maintain a live Advanced Clinical Practice Portfolio
- Approved Clinician Status
- A willingness and commitment to achieve Approved Clinician Status within an agreed timeframe
- Extensive post registration clinical experience in advanced practice within speciality area
- Evidence of performing at Advanced Level of Practice
- Proven track record of working collaboratively with multi-disciplinary teams
- Experience and knowledge of research processes and being involved in research practice
- Experience of teaching within a wide multi-professional audience, both in the clinical and University setting
- Experience of leading services and teams
- Experience of change management
- Experience of clinical policy and guideline development and implementation at local and national level
- Experience of leading on and implementing research within clinical practice
- Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
