- Assess, plan, develop, implement and evaluate programmes to promote health and wellbeing.
- Implement and evaluate individual treatment plans for patients with a known long term condition.
- Proactively identify and manage as appropriate, treatment plans for patients at risk of developing a long term condition.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
- Support patients to adopt health promotion strategies that encourage patients to live healthily.
- Provide information and advice on prescribed, or over the counter medication on medication regimens, side effects and interactions.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognise, assess and refer patients presenting with mental health needs.

- Current NMC registration with an active PIN.
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
- Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.

- Knowledge, and experience of its application, in supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.
- Long term conditions experience desirable.
