– Manage chronic diseases such as asthma, COPD, and diabetes, ensuring thorough patient monitoring and support.
– Provide core nursing services in general practice, conducting clinics for long-term conditions, immunisations, cervical cytology, and contraception.
– Deliver wound care, assist in minor surgeries, and perform ear care, ECGs, and venepuncture.
– Carry out new patient screenings, aid in chronic disease monitoring, and participate in health promotion activities.
– Undertake home visits, offering a supportive role to GPs and senior nurses, fostering a collaborative approach to patient care.
– Engage in clinical governance, quality improvement, and safeguarding, maintaining high standards in all practices.
