- Conduct needs assessments under the Care Act 2014 to determine support requirements.
- Work with service users, their families, and carers to ensure they receive appropriate support.
- Assist with accessing housing, employment, education, and community resources.
- Respond to emergency situations, such as a breakdown in care or safeguarding concerns.
- Work within the Care Act 2014, Mental Capacity Act 2005, and Deprivation of Liberty Safeguards (DoLS) framework.
- Advocate for peoples rights, ensuring their voice is heard in care decisions.
- Collaborate with health professionals, psychologists, occupational therapists, housing services, and advocacy groups
- 1-3 years experience of working within a similar social work setting
- Diploma/Degree in Social Work
- Registration with Social Work England
- Experience of working with people who have a learning disability, have care act, mental capacity act and best interests experience alongside risk management and multiagency working
