We are looking for an enthusiastic and flexible Care Navigator to join our team and to contribute in providing excellent patient care.As a Care Navigator, you will play a pivotal role in delivering patient care and services from the first encounter to our diverse patient population. You will be well supported by an established team to provide comprehensive support and manage day to day interactions as you will be a focal point for our patients in accessing appropriate heath care.Main duties of the jobTo provide comprehensive administrative support for the general practice and to act as focal point for patients and staff.To receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective wayOffer general assistance to the practice team and project a positive and friendly image to patients and other visitors either in person or via the telephone.Facilitate effective communication between patients and members of the healthcare community.Ideally we would seek a candidate with primary care experience however to facilitate patient access via AskmyGP and SystmOne - training will be given.About usHaven Health is a high-achieving, forward-thinking practice that fosters a positive and inclusive workplace.   • GCSE grade A to C in English and Maths
  • One years experience in busy customer service or reception environment.
  • Experience of working in a team.
  • IT literate preferably confident using microsoft Office
  • Understanding of health and safety policies and procedures.
  • Ability to translate information and details accurately, both verbally and in writing.
Desirable
  • Previous NHS experience
  • Experience of clinical systems, including SystmOne.
