We are a welcoming, proactive practice based in a modern, well-equipped premises, with an enthusiastic and supportive team.Our friendly reception staff and dedicated nursing team work closely alongside two full time GP's and two advanced Nurse Practitioners creating a collaborative and positive environment for both staff and patients.Main duties of the jobProvide general medical services to patients registered with the practice.Undertakea range of consultations including face to face, telephone, online, and home visits where appropriate.Assess, diagnose, treat and manage patients medical conditions. Request, interpret and act on diagnostic tests and investigations.Provide appropriate clinical follow-up and referrals to secondary care when required.Provide health education, preventative advice, and promote healthy lifestyles.Support the management of patients with chronic disease and long -term conditions.Work collaboratively with the practice nursing and administrative teamsContribute to QOF,IIF , PCN and enhanced services.Participate in Practice meetings and contribute to service development.Maintain accurate up to date and legible clinical records.Review action and sign repeat prescriptions and test results.Complete medical reports, forms, and insurance documents in a timely manner.About usWe are a friendly, proactive and forward thinking practice .   • Previous experience of working in UK General practice
