We are seeking an experienced ANP/ACP to join our team. We operate out of two sites and provide primary care services to approximately 11,500 patients. They will provide care for the presenting patient, including initial history collection, clinical assessment, diagnosis, treatment, and care evaluation. They will provide safe, clinical decision-making and professional treatment for patients in general practice.Hours negotiable, ideally 24-37 hours.Main duties of the jobThe Advanced Clinical Practitioner (ACP) / Advanced Nurse Practitioner (ANP) is an experienced healthcare professional who provides high-quality, patient-centred care within the general practice setting. Working autonomously within their professional boundaries, the ACP is responsible for the assessment, diagnosis, treatment, and evaluation of patients presenting with undifferentiated and undiagnosed conditions, as well as those with chronic illnesses.The post holder will undertake comprehensive clinical assessments, take patient histories, and make safe, evidence-based decisions regarding treatment and care plans. They will exercise advanced clinical reasoning and professional judgement to deliver effective care, ensuring a high standard of service that supports both the immediate and long-term needs of the practice population.This role requires working both independently and collaboratively as part of the multidisciplinary team to ensure continuity, safety, and excellence in patient care within the primary care environment.If the post-holder is an ANP, they must have a digital badge for advanced practice.About usChurch Road Surgery and Tile Cross Surgery operate as a single GP organisation and are part of Our Health Partnership.
