
Assistant Practitioner - Discharge Liaison

- To work with MPFT community colleagues, social partners, voluntary sector, care/nursing/residential homes, patients and their carers to proactively support and facilitate timely and safe discharges from hospital to home or onward care settings.
- To act as the assistant practitioner in discharge liaison on Adult and Old age wards at the Redwoods Centre to co-ordinate safe and timely discharges to prevent delayed transfer of care under the close supervision of the Lead Site Manager.
- To work closely with Care coordinators and social services to screen and support self-funding patients (and their families/carers) with discharge planning under the direction of ward managers and the ward team.
- To contribute to the MDT to have an up-to-date discharge plans in place allowing an optimum time to achieve discharge dependent actions to reduce any potential transfer of delays.
- To work in partnership with the MDT, social services, clinical commissioning groups, voluntary agencies and next of kin regarding simple and complex needs identified during the admission process to facilitate timely discharge under the supervision of clinical matron.
- oExperience of working with people with Mental Health Needs as a member of a multi-disciplinary community team
- oExperience of working within an Inpatient setting
- oDiploma in Health and Social Care or NVQ level 3 or equivalent.
- oFoundation Degree in Mental Health with associated competences
- oExperience of working with people with Mental Health Needs as a member of a multi-disciplinary community team
