Do you have experience of working with people with Mental Health needs? We are looking for a motivated enthusiastic person in the role of Discharge Liaison Practitioner to join the Crisis Home Treatment Service in ShrewsburyYou will be required to work closely with the acute and older adult in-patient ward multidisciplinary teams to ensure the safe effective and timely organisation of patient discharges. Within this role you will also liaise with the wider patient flow team, social services, bed managers, third sector providers and Crisis/Home treatment teams.You must have good communication skills and be organised in your approach to work.The primary focus of the Discharge Liaison role is to enable and support the safe and timely discharge of patients. You will need to consider the options around a patient's discharge and take appropriate action to resolve delays in discharge. This will ensure capacity is created in the system to allow the wards to have empty beds to match their demand. The key element of the Discharge Liaison role is to communicate with the bed manager and the nurse in charge to ensure everyone is working to the same plan of no delays for patients.Main duties of the job
  • To work with MPFT community colleagues, social partners, voluntary sector, care/nursing/residential homes, patients and their carers to proactively support and facilitate timely and safe discharges from hospital to home or onward care settings.
  • To act as the assistant practitioner in discharge liaison on Adult and Old age wards at the Redwoods Centre to co-ordinate safe and timely discharges to prevent delayed transfer of care under the close supervision of the Lead Site Manager.
  • To work closely with Care coordinators and social services to screen and support self-funding patients (and their families/carers) with discharge planning under the direction of ward managers and the ward team.
  • To contribute to the MDT to have an up-to-date discharge plans in place allowing an optimum time to achieve discharge dependent actions to reduce any potential transfer of delays.
  • To work in partnership with the MDT, social services, clinical commissioning groups, voluntary agencies and next of kin regarding simple and complex needs identified during the admission process to facilitate timely discharge under the supervision of clinical matron.
