We are seeking an experienced and organised Care Coordinator to join North Birmingham Primary Care Network (PCN), working solely at Shah Zaman Surgery. This part-time role offers 16 hours per week across 3 days. As the lead for all patient recalls, you will play a central role in ensuring patients receive timely and coordinated care, supporting delivery against QOF and the Universal Offer. Experience of using SystemOne is essential, alongside an understanding of recall processes in primary care. You will also contribute to safeguarding, Gold Standards Framework (GSF) work, and provide valuable support to the Practice Manager. Joining our team means playing a key role in strengthening the link between patients, clinicians, and wider services, making a real difference to the health of our local community.Main duties of the jobThe Care Coordinator will act as the lead for all patient recalls at Shah Zaman Surgery, ensuring that QOF and Universal Offer requirements are met. This will involve proactive management of recall lists, scheduling, and follow-up to support patients in accessing appropriate care in a timely way. In addition, the post holder will support safeguarding processes, including organising meetings, taking accurate minutes, maintaining the safeguarding register, and working closely with the Safeguarding Lead. They will also assist with GSF coordination and provide administrative support to the Practice Manager as required. This role requires excellent organisational and communication skills, with the ability to liaise effectively with patients, clinicians, and wider healthcare professionals.
