– Develop and implement tailored treatment plans based on insightful clinical reasoning.
– Employ evidence-based interventions to enhance ADLs (Activities of Daily Living), cognitive functioning, and physical abilities among patients.
– Facilitate smooth discharge planning through effective home visits and making referrals as necessary.
– Provide insightful equipment recommendations and necessary adaptations to ensure patient safety and independence.
– Maintain thorough clinical records compliant with HCPC/NHS standards.
– Collaborate seamlessly with medical staff, nurses, physiotherapists, social workers, and discharge coordinators for optimal patient care.
– Actively participate in MDT reviews, ward rounds, and case discussions.
– Mentor and supervise junior staff, students, and support workers to foster a thriving team environment.
– Play a role in service development and audit projects, contributing to the enhancement of service delivery.
