- Your role will be pivotal in implementing effective care plans that enhance patient autonomy and quality of life.
- Coordinate with multidisciplinary teams to ensure comprehensive patient care.
- Conduct access visits, requiring a car and valid driving license, ensuring patients have a safe and supportive environment.
- Work independently at Inpatient settings and integrate seamlessly with different healthcare units including PTC, CU27, and AMH.Requirements:- Registered with the Health and Care Professions Council (HCPC).
- Practical experience in inpatient settings with a focus on discharge planning.
- Proficiency in RIO is desirable.
