– Be adept at carrying out duty and welfare checks with Home First to facilitate seamless discharges from D2A beds.
– Complete Care Act 2014 assessments and other necessary evaluations such as Mental Capacity Act Assessments and Continuing Healthcare Checklists.
– Actively participate in CHC Decision Support Tool meetings, contributing your expertise to ensure comprehensive care planning.
