We are seeking a Practice Nurse to join our nursing team. The post-holder will support patients in managing their health, promote wellbeing, and help prevent illness, in line with practice protocols, current evidence-based guidelines, and the NMC Code of Professional Conduct. Key Responsibilities ClinicalUndertake holistic assessment, planning, implementation, and evaluation of nursing care.Provide chronic disease management clinics (e.g. Asthma, COPD, Diabetes, Hypertension, CHD, Epilepsy, Mental Health, Rheumatoid Arthritis).Deliver immunisations and vaccinations (childhood, adult, seasonal, and travel).Perform cervical cytology screening in line with national guidelines.Provide wound care, dressings, suture removal, and management of minor injuries.Conduct health screening and health checks (e.g. new patient checks, NHS health checks, vascular risk assessments).Support patients with long-term conditions, encouraging self-management and lifestyle change.Assist GPs with minor surgery procedures when required.Carry out diagnostic tests and monitoring (e.g. ECGs, peak flow, spirometry, blood glucose, INR).Deliver travel health advice and vaccinations, supporting the practice travel clinic.Provide contraceptive and sexual health advice within scope of practice.Identify mental health concerns and signpost or refer to appropriate services.
