Formulate diagnoses and treatment plans, utilising multidisciplinary skills where competent, and adopting a "what matters to me" holistic approach.Facilitate safe and effective discharges from the inpatient setting, working collaboratively with the multidisciplinary team (MDT) to enable more patients to be at home.Provide leadership to support staff and junior team members through supervision, training, and appraisal.Contribute to the planning and development of Physiotherapy Services and the End of Life model of care within the designated team.Demonstrate flexibility in working across both Inpatient and Community settings as service needs require.About usWe're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly.
