The independent assessment, treatment and discharge planning of patients in community rehab team. Using recognisable outcome measures and clinical reasoning to guide treatment plans. Confident to plan own caseload and treatment plans. Experience in NHS and post qualification as an Physiotherapist, preferably within the community and rehab setting. Car drivers only.To provide specialised Physiotherapy assessment, education, advice and treatment to adult patients who have complex health and social care needs; To work as a member of a multidisciplinary team to support hospital discharges, provide rehabilitation in the patients' home environment and reduce care needs; To independently manage a defined caseload of complex cases contributing to the provision of a rehabilitation service; To provide a high quality, efficient therapy service to patients with a wide range of physical and medical conditions referred to the Maximising Independence service line.It is essential for applicants to have a minimum of 1-year post-graduate experience, right to work in the UK and HCPC registration.When you work with JustPhysio, you will have access to a range of benefits that will support you in your role.
