
Senior Community Outreach Worker

- Engaging with rough sleepers across London who are affected by drug and/or alcohol use
- Conducting needs and risk assessments in-person
- Delivering trauma-informed, harm reduction support and brief psychosocial interventions
- Advocating and referring into local statutory substance misuse services
- Case manage clients independently while collaborating within a small, experienced, and supportive team
- Navigating multi-agency partnerships to ensure holistic, coordinated care
- Working flexibly and proactively engaging hard-to-reach clients to improve health outcomes, quality of life, and access to treatment
- Experienced in supporting people with substance use and/or homelessness issues (essential)
- Skilled in trauma-informed care and brief psychosocial interventions
- Knowledgeable about harm reduction practices concerning substance use
- Confident with risk management, safeguarding, and lone working protocols
- Able to work independently, manage a complex caseload, and prioritise in a dynamic outreach environment
- Empathetic, adaptable, and confident communicating with diverse communities and professionals
- Willing and able to travel across London regularly (expenses covered) including early, late, or weekend work
- Starting salary of £30,836 annum, with opportunity to access potential yearly salary increments subject to appraisal
- 25 days annual leave plus Bank Holidays
