  • Engaging with rough sleepers across London who are affected by drug and/or alcohol use
  • Conducting needs and risk assessments in-person
  • Delivering trauma-informed, harm reduction support and brief psychosocial interventions
  • Advocating and referring into local statutory substance misuse services
  • Case manage clients independently while collaborating within a small, experienced, and supportive team
  • Navigating multi-agency partnerships to ensure holistic, coordinated care
  • Working flexibly and proactively engaging hard-to-reach clients to improve health outcomes, quality of life, and access to treatment

  • Experienced in supporting people with substance use and/or homelessness issues (essential)
  • Skilled in trauma-informed care and brief psychosocial interventions
  • Knowledgeable about harm reduction practices concerning substance use
  • Confident with risk management, safeguarding, and lone working protocols
  • Able to work independently, manage a complex caseload, and prioritise in a dynamic outreach environment
  • Empathetic, adaptable, and confident communicating with diverse communities and professionals
  • Willing and able to travel across London regularly (expenses covered) including early, late, or weekend work

About Phoenix FuturesPhoenix have over 50 years’ experience of delivering pioneering services. This provision, part of a newly commissioned Pan-London Substance Use Pathway, will combine our considerable expertise in substance use and homelessness.
