Responsibilities:
  • To provide social care to people who have moderate to severe mental health problems across a wide range of presenting conditions.
  • You will be supporting the council's obligation to the Care Act, completing assessments, funding applications, copdols, 21a challenges and other statutory responsibilities as well as providing mental health support to the residents of Tameside.
  • You will also work with children that are turning 18 and transitioning from children's services that has a Mental health need.
  • You will be responsible for the co-ordination, commissioning and personal delivery of individual packages of care & treatment for a caseload of service users within our secondary mental Health service.
Requirements:
  • Social Work England registered
  • Social Work Degree or equivalent
  • Enhanced DBS
