- Act as the first point of contact for referrals from individuals, families, health professionals, and partner agencies.
- Conduct timely initial assessments to identify risks, needs, and eligibility for adult social care services under the Care Act 2014.
- Screen and assess safeguarding concerns, responding promptly to allegations of abuse or neglect.
- Initiate safeguarding enquiries under Section 42 of the Care Act and ensure compliance with safeguarding procedures.
- Provide advice, information, and guidance to individuals and carers, linking them to community resources, preventative services, or voluntary organizations.
- Promote independence and well-being by supporting access to early intervention services.
- Respond to urgent and crisis situations, ensuring individuals are safe and immediate needs are addressed.
- Work with emergency services, health partners, and family members to stabilize situations effectively.
- Use a strength-based approach to empower individuals and their carers, focusing on what they can achieve with the right support.
- Conduct proportionate assessments to create personalized care plans and outcomes.
- Apply the Care Act eligibility criteria to determine entitlement to care and support services.
- Make recommendations for further involvement from longer-term social work teams or specialist services.
- Work closely with health services, housing, voluntary sector organizations, and other local agencies to ensure a holistic approach to support.
- Attend and contribute to multidisciplinary meetings to develop coordinated plans for individuals.
- Undertake mental capacity assessments and best interest decisions where required.
- Ensure compliance with the Mental Capacity Act 2005 and associated Deprivation of Liberty Safeguards (DoLS).
- Maintain up-to-date, accurate, and detailed case records using the council's case management system.
- Ensure all records meet legal, organizational, and professional standards.Focus on preventative strategies to delay or reduce the need for more intensive care and support.
- Promote access to aids, adaptations, and technology-enabled care to support independence.
- Ensure appropriate allocation of resources and services based on needs and available funding.
- Work within budgetary constraints while advocating for the best outcomes for individuals.
- Attend training, supervision, and team meetings to maintain professional development and adhere to Knowsley Council's policies and procedures.
- Keep up to date with legislation, policy changes, and best practices in adult social care.
- Contribute to service monitoring, audits, and quality assurance processes.
- Ensure timely progression of cases and adherence to response timeframes for incoming referrals.
