An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse to join our existing team, providing Adult Diabetes Services across the Havering area for three days per week.Our Integrated service model encompasses a multidisciplinary approach to Diabetes Care in the community setting, allowing shared responsibility for the person with diabetes. You should submit your interest and apply through the Capital Nurse London group.Main duties of the jobThe successful candidate would be expected to work within a team of skilled diabetes specialist nurses and to deliver a high standard of evidence based clinical care to patients and their carers, and to work as part of the Integrated Diabetes Service, within the community setting. The successful candidate must be confident to deliver education sessions to both patients and healthcare professionals.The successful candidate will require excellent communication skills and will need to be confident to work alone and within a multidisciplinary team.We are seeking a highly motivated individual who will contribute to the developments of our multi-disciplinary specialist community service and who has a commitment to continued personal development and is able to travel efficiently across the area of Havering , including to patients in their own homes.About usProbationary PeriodThis post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.
