- Triaging patient data and referrals with clinical precision
- Coordinating care with GPs, paramedics, and community teams

- Experience in unscheduled care and team leadership
- Knowledge of frailty, heart failure, COPD, and diabetes
- Excellent communication, documentation, and IT skills
- A passion for innovation and continuous professional development
- Registered General Nurse or Paramedic - NMC or HCPC
- Evidence of post registration experience
- Mentorship course or ENB 998 (teaching and assessing course) or must be willing to undertake within 1 year)
- Educated to degree level
- Postgraduate qualification in health-related subject
- Triage experience
- Evidence of well-developed critical decision-making skills
- Ability to manage a team
- Understanding of the Health Care Agenda
- Demonstrates commitment to and recent evidence of further education and professional development
- Ability to deal with a wide range of people at all levels including clients, carers, therapists, health and social care colleagues and other professionals
- Demonstrates a positive attitude towards change
- Understanding of multi-disciplinary working in the care of adults
- Knowledge of clinical governance
- Understanding of the Single Assessment Process
- Experience of unscheduled care
- Experience of supervising, coaching, supporting, and mentoring less experienced staff
- Experience of clinical supervision

- Ability to work flexibly within a shift pattern
