City Health Care Partnership CIC are looking for highly motivated, enthusiastic individuals to join the developing team of practitioners based across Hull and East Riding.The post holder will be responsible for the assessment, diagnosis and treatment of urgent same day need, and primary care patients. Presentations include undiagnosed undifferentiated illness, minor injuries, and also exacerbation of long-term conditions. Consultations are performed via face to face, telephone, or a digital platform.There is an expectation to work collaboratively with partner organisations, ensuring that care is seamless and timely to ensure the best possible patient experience. You will also promote exceptional standards of patient care underpinned by the implementation of evidence-based practice, audit and regarding current available guidelines and research.The post holder will be an experienced clinician who will act within their professional boundaries and will support the delivery of policies and procedures. They will act as a positive role model supporting junior staff whist working flexibly to provide safe clinical expert care.Main duties of the jobEffective leadership skills will be required, and the successful candidate will be one who is motivated, dedicated, and enthusiastic. Working as part of an established clinical workforce, you will benefit from a multi-disciplinary team and be exposed to an extensive range of services and healthcare settings and locations. You will work alongside our newly developed multi-professional clinical model and be supported by a network of clinicians to share expertise. Post holders will initiate clinical assessment including history-taking, clinical examination, diagnosis, treatment and referral, using a range of advanced skills and expert knowledge for patients of all ages presenting with undifferentiated and undiagnosed conditions.
