If you're passionate about helping individuals live in their own homes and thrive, we want to hear from you.π What Weβre Looking ForWeβre seeking a driven and experienced professional who is:Qualified to Degree level (or equivalent) in a relevant field (e.g. RNLD, RNMH, Social Work, Health and Social Care, Psychology, OT)Experienced in supporting people with learning disabilities, autism, and/or mental health needsSkilled in assessment and transition planning within health & social careAdept at building strong professional relationships with individuals, families, and key stakeholdersConfident in managing multiple referrals, deadlines, and priorities with a solution-focused mindsetAble to travel across the Midlands and North West (overnight stays required occasionally)ποΈ A Day in the Life of a Referral and Assessment ManagerYouβll be the key contact for new referrals β assessing individual needs, designing tailored transition plans, and collaborating with operational teams to implement high-quality support.
