Team: Integrated Discharge Service, Occupational Therapy & Sensory ServiceJob Description:Liquid Personnel is recruiting Occupational Therapists for its clients Integrated Discharge Service; Occupational Therapy & Sensory Service Team located at Hackney.Join our client’s dynamic Hospital Team delivering high-impact occupational therapy interventions that promote independence and support safe, person-centred hospital discharges. This is a hybrid role with opportunities to work across London hospitals and in the community.What will your responsibilities be?
- Assess Hackney residents in hospitals across London for safe discharge planning
- Collaborate with multi-disciplinary teams and attend ward meetings
- Support residents with complex needs, including LD, MH, dementia, and NRPF
- Conduct community reviews and Care Act assessments
- Promote co-production and independence in daily living
- Weekend working optional
- Inclusive, integrated team culture
- Focus on holistic, person-centred outcomes
- HCPC-registered Occupational Therapist
- Experience in hospital or social care settings
- Strong assessment and Care Act review skills
- Knowledge of safeguarding, MCA, and discharge planning
- Excellent communication and problem-solving abilities
