We are seeking an enthusiastic Occupational Therapist to support residents transitioning home from hospital, enhancing their independence and achieving their personal outcomes. What you will do:
  • You will be supporting the Discharge to Assess pathway by screening referral documentation. Your decision-making prowess will ensure seamless planning and support for people returning home.
  • Conduct post-discharge assessments in clients' homes, offering therapy interventions to promote recovery and independence or prevent re-admissions. Recommend environmental adaptations, equipment, or alternative methods to accomplish daily activities.
  • Collaborate with clients to develop personalised Reablement Plans, focusing on strengths and outcomes, whilst embracing a flexible working environment that combines home and office-based tasks.
  • Additionally, you must work effectively with multidisciplinary teams, contributing to the full breadth of discharge pathways and goals.
