We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.Within the Older Adult Care Home Liaison Service,The post holder will provide mental health nursing advice, support and education to identified Care Homes within South CHCP. They will work closely with Multi-Disciplinary colleagues within the older adult's service and be responsible for assessing the mental health needs of clients who are referred from the Care Homes. Liaising with local community services and working closely with the clients RMO (GP) to agree and co-ordinate care packages. The post holder will provide on-going care management ensuring care plans are regularly reviewed with an aim to maintain the clients level of ability and prevent hospital admission. They will link with the hospital discharge service to facilitate earlier discharges which follow the identified care pathway. The post holder will also be responsible for managing their own caseload and ensure effective communication between Care Homes and the local community older adult services.
