  • Foster independence and empower individuals in line with current legislation, while aligning with the policy and practice of the local authority.
  • Collaborate effectively with service users, their carers, and other professionals to develop tailored and outcome-focused care plans.
  • Remain proactive in the management of cases, responding adeptly to dynamic circumstances and changing needs.
