- Assess, plan, implement and evaluate individualised programmes of care.
- To receive and communicate complex, sensitive or contentious information to patients and carers, who may have barriers to understanding eg dysphasic, complex mental health problems, to maximise rehabilitation potential and to ensure understanding of conditions.
- To have expert verbal and non-verbal communication skills of motivation, explanation and reassurance to agree and gain co-operation of patients and carers in their treatment programmes.
- To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of rehabilitation and to ensure a consistent approach to patient care and treatment e.g. moving and handling.
- To provide nursing advice, teaching and training to other members of the MDT regarding the management of patients and the role of nursing.
- To train, supervise and performance manage junior staff, senior assistants and assistants and students in a clinical, professional and organisational capacity. This may include undertaking formal appraisal.
- To assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.
- To be responsible for ensuring the effective selection and use of all treatment resources.
- Registered General Nurse
- Evidence of post basic education at Diploma/Degree level
- Demonstrate commitment to continuous Professional Development
- Evidence of relevant post graduate development in respiratory &/or cardiac care
- Trained in Basic Life Support
- To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
- Degree in health related subject
- ENB qualification or equivalent
- Immediate Life Support trained
- Postgraduate Certificate
- Post-registration experience and experience in managing respiratory, cardiology /cardiac conditions
- Experience of supervising / managing / developing Pre- registration nurses
- Experience of Audit
- Experience in delivering cardiac or pulmonary rehabilitation
- Experience of working in community setting (pre or post registration)
- Involvement in research
- Experience or involvement in service development
- Knowledge of the principles of Clinical Governance and its application
- Knowledge of National Guidelines for cardiac and pulmonary rehab
- Specialist understanding of cardiac / respiratory conditions, pathways and interventional procedures
- Commitment to professional Development
- Excellent interpersonal skills
- Analytical Clinical assessment, treatment and diagnostic skills relevant to this clinical area

- Due to the requirement to undertake home visits, the post holder is required to be either a car driver with a valid driving licence
