Full Time / 37 Hours Per Week / Fixed Term 12 monthsA unique opportunity has arisen for a qualified OT to join our Home First Service. The Home First Service is made up of our co-located services from across Adult Social Care and Community Health (Nursing/Therapy Teams). The service enables timely hospital discharges for those needing care and support by avoiding the risk of de-conditioning whilst optimising their independence. The Home First OT will work closely with the Reablement Service (which supports the majority of hospital discharges) and the Integrated Crisis Response Service (which supports hospital avoidance) and will ensure our intervention is strengths based and person centred. The OT within this role will work collaboratively within the co-located multi-disciplinary team ensuring people have the opportunity and support to maintain and maximise their independence with a view to avoid admission/ re-admission to Hospital and a reliance on long term care.This role will be based at the Neville Centre (on the Leicester General hospital site, with your own designated car parking) working mainly core office hours 8:30am to 5:00pm Monday to Thursday and 8:30am to 4:30pm Friday. However, there will be flexibility and autonomy within this role to help manage weekly commitments and help provide support out-of-hours and weekends too.What you’ll be doingAs an OT within the Home First Service, you will be completing assessments within the person’s home environment, with intervention based around their needs, what is important to them and goals they wish to achieve. You will be required to assess, and prescribe appropriate equipment and recommend adaptations. You will be expected to work collaboratively with the Community Health Therapy Team, Reablement/ICRS Service, and the Reablement Social Work Team, ensuring effective communication and an integrated and co-ordinated approach at all times. The role will also include providing a level of therapeutic support for people who may have become de-conditioned or have reduced confidence.What you’ll needAs a Qualified Occupational Therapist who is registered with HCPC you’ll be committed to a strengths-based approach and positive risk taking. You’ll have excellent interpersonal skills, meaning you can work well alongside people in complex situations and circumstances. Your passion to make a positive change will mean that you can show ability to understand and value the differences in the diverse groups of people in Leicester City and develop solutions for people and families that meet individual needs. As a valued member of a busy Home First Service, you’ll have good time management skills and be able to prioritise between competing demands.Due to the nature of this role, you’ll be subject to an Enhanced Adults and Children’s DBS.
