This full-time role offers a competitive salary of £36 per hour over a 36-hour week, initially on a 3-month contract with the potential for extension. You will be working closely with the esteemed Alice Fayenuwo, your line manager, as part of the Hayes and Five Elms Locality Team. Our team is at the forefront of providing ongoing care management interventions and is deeply involved with annual reviews of support plans, extra care housing, and direct payment work. You’ll also be delivering assessments under the Care Act 2014 and assisting young people transitioning from Children’s services to Adult services.   • Support Younger Adults with physical disabilities and sensory impairment to live independently.
  • Collaborate with carers to manage associated risks effectively.
  • Participate in ongoing care management interventions.
  • Lead annual reviews of support plans, extra care housing support, and direct payment work.
  • Conduct assessments under the Care Act 2014.
  • Facilitate the transition from Children’s services to Adult services.
  • Work within a multidisciplinary framework, communicating and negotiating with partner agencies.
  • Undertake safeguarding work and Best Interest assessments as a duty senior on a rota basis.
  • Provide supervision for both unqualified and qualified Care Managers.
