Engage in meaningful interventions and lead the charge on support plans, extra care housing support, and direct payments. Your work will ensure these individuals live independently while helping to manage risks effectively. – Manage ongoing care interventions and coordinate annual reviews of support plans.
– Work collaboratively within a multidisciplinary framework to support individuals’ independence.
– Develop strong relationships with partner agencies to ensure a cohesive approach to care.
– Oversee safeguarding duties and conduct Best Interest assessments as needed.
