- Conduct comprehensive assessments to identify clients’ needs, strengths, and support requirements.
- Develop and implement personalised care plans in collaboration with clients and their families.
- Provide guidance and support to clients, assisting them in accessing resources and services.
- Work closely with a range of professionals including healthcare providers, therapists, and support staff to ensure a holistic approach.
- Monitor and review client progress, adjusting care plans as necessary to ensure the best outcomes.
