
Practice Nurse -Diabetes - 30 hours per week

- Assist in and perform routine tasks related to patient care
- Carry out diabetes reviews and contribute to long-term condition care planning
- Monitor insulin and other diabetes medications as part of patient management
- Provide diabetes education and clinical guidance to the wider primary care team
- Run general nursing clinic, including venepuncture, ECGs, BP monitoring, wound care, and health screening
- Collaborate with the wider clinical team to ensure continuity of care
- Contribute to minor illness and general treatment clinics (if qualified)

- Assist in and perform routine tasks related to diabetic patient care as directed by recall, diabetic nurse colleagues and GPs
- Undertaking reviews for those patients with Diabetes as part of Care planning for Long Term Conditions.
- Monitoring the dose of those patients taking insulin and other diabetic medication.
- Share diabetes education and guidance to the wider primary care team
- Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs.
- Minor Illness (if qualified) and General clinics
- Basic wound care / ulcer care
- Removal of Sutures
- Urinalysis
- ECGs
- Venepuncture
- Blood pressure measurements
- Chaperoning and assisting patients where appropriate who are being examined by another clinician
- Requesting pathology tests
- Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
- Other tasks including:
- Maintaining and cleaning equipment used by the nurses and GPs
- In the absence of an HCA, monitoring the temperature of clinical fridges, to ensure adherence to the Cold Chain Policy.
- Liaise with nursing colleagues to ensure that regular checks and testing of emergency equipment and drugs are performed and documented.
- Undertake housekeeping duties including, general tidiness and cleanliness of nurses and treatment rooms
- Participation in administrative systems in the Practice
- At all times there will be a need to maintain accurate records
- A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
- Attend and participate in any Practice meetings when required.
- Completion of mandatory training
- Any other delegated duties appropriate to the post.

- Evidence of a commitment to continuing professional development,
- Relevant nursing/health degree level modules,
- Diabetes Qualification
- Minimum 2 years post registration diabetic experience,
- Mentor/teaching qualification,
- Have or be working towards the Independent Prescribing Course,completion.
- Previous experience in the management of diabetes,
- Knowledge of needs of patients with long-term conditions,
- Aware of accountability of own role and other roles in a nurse led service,
- Knowledge of health promotion strategies,
- Awareness of clinical governance issues in primary Care,
- Ability to identify determinants on health in the local area,
- EMIS experience.
- At least 1 year recent primary and community nursing experience,
- Management of long-term Conditions,
- Involvement in implementing and using protocols and clinical guidelines,
- Experience and involvement in Diabetes Patient Education Programs,
- Participation in quality initiatives such as clinical benchmarking,
- Experience of audit
- Registered first level nurse,
