
Housing & Communities Connector

- Provide one-to-one support to residents, including home visits and advocacy
- Help residents navigate health, social care, and community services
- Co-produce personalised wellbeing and tenancy plans
- Link people to local activities, groups, and opportunities
- Work closely with CHS teams to address housing-related challenges
- Monitor progress and share learning to improve services
You’ll be a natural connector with the resilience, empathy, and organisational skills to support people with complex needs. You’ll be confident working both independently and as part of a team, and comfortable visiting residents in their homes.You’ll bring:
- Experience supporting vulnerable people or those facing social exclusion
- Strong communication and relationship-building skills
- Ability to assess needs and manage risks appropriately
- Knowledge of safeguarding good practice
- Basic IT skills (Word, Excel, email)
- A Level 3 qualification (or equivalent experience) in a relevant field
- A full UK driving licence and access to a car
