
Night CQC Reablement Worker

- Working closely with the care team to follow support plans, reporting concerns.
- Help customers regain or maintain independence to live well in their own home by carrying out every day daily living activities themselves.
- Carrying our risk assessments, support planning, goal setting and regular reviews.
- Supporting customers who have more complex needs with personal care including bathing, toileting and getting dressed.
- Supporting our customers in their own apartment who have a range of support needs including frailty and dementia.
- 34 days leave, pro-rated (including bank holidays and a “me day”)

- A level 2 or above, or equivalent in social care or being willing to work towards this
- Care Certificate or willing to complete within the first 12 weeks of employment
- If you’ve worked in a CQC care setting previously, that would be great!
- Have a positive, caring can-do attitude, and empathy to really make a difference to our customers lives.
- Previous experience of working with support plans, recording and using information concisely and effectively.
- Knowledge of local community and networks would be brilliant.
- We provide 24-hour support.
- Night shifts with some lone working, plus regular team meetings and training.
- Core shifts are Thursday, Friday and Saturday evenings.
- Occasionally our customers' needs mean your start and finish times may change. Where we ask you to be flexible, we will always try to reciprocate
- Confident using technology for plans, online learning and teamwork
- You’ll need an Enhanced DBS check done and we pay for that.
