Hospital Discharge | Adults Team Manager | 4 Days On SiteStockport Metropolitan Borough Council (SMBC) is seeking a strong and dynamic Team Manager to lead their ARCH Hospital Discharge Team based at Stepping Hill Hospital. This is a vital leadership role, coordinating safe and timely discharges across a multi-disciplinary team within a high-pressure environment.You’ll need to be on-site at least 4 days per week, splitting time between Stepping Hill Hospital and Regent House. This is a hands-on post requiring excellent knowledge of adult social care legislation and experience managing large and diverse teams.Key Responsibilities:Lead and supervise a large team including Social Workers, ASYEs, and apprenticesManage hospital discharge pathways and promote safe, effective transitionsEnsure compliance with the Care Act, MCA, DoLS, and safeguarding frameworksChair safeguarding meetings, COP work, MDTs, and inter-agency meetingsMaintain operational flow and staff support in a fast-paced hospital settingCollaborate with partners across ICBs, Health, and the third sectorPractice Model:
You will work closely with internal and external professionals to manage adult discharge planning across multiple pathways. This role demands strategic oversight, confident supervision, and resilience under pressure.Essential Requirements:Qualified Social Worker registered with Social Work EnglandMinimum 3 years’ experience as a Team Manager in adult servicesIn-depth knowledge of adult social care legislation and best practiceExperience leading multi-disciplinary teams in a hospital or discharge settingMust be able to work on-site 4 days per weekFull UK Driving LicenceBenefits:Up to £46 per hour (DOE)8:30am–5pm working hoursSupportive, high-performing team in a critical service areaMeaningful leadership in adult hospital discharge and safeguardingInterested?
