We are looking for a dynamic and adaptable team manager with at least 3 years of experience who can handle the pressures of a fast-paced environment.   • Lead and supervise a dedicated team of social workers and unqualified staff in the Hospital Discharge (ARCH) Team.
  • Ensure compliance with key legislation including the Care Act, safeguarding, and Mental Capacity Act.
  • Manage and support hospital discharge processes, enhancing patient outcomes and operational flow.
  • Collaborate extensively with integrated care boards, health professionals, and third-party sector partners.
  • Chair safeguarding meetings and participate in multidisciplinary team discussions.
  • Oversee and develop the team to enhance their performance, inspiring them to achieve their best in supporting vulnerable adults.
