- Lead the implementation and oversight of complex care packages in the community
- Complete clinical assessments for new clients and ongoing care reviews
- Deliver clinical supervision, training, and competency assessments for nursing staff
- Conduct medication and compliance audits to ensure safe, high-standard care
- Collaborate with multidisciplinary teams to coordinate holistic care plans
- Provide both routine and emergency clinical support to regional care teams
- Competitive salary up to £42,000 per annum
