staff and volunteer team, who are as passionate and committed as we are to meeting the needs of our local communities.As our Housing First Worker, you will be responsible for:
- Co-producing support plans and implementing appropriate support to address the needs of vulnerable homeless people presenting with mental health, substance misuse, offending behaviour and the particular challenges faced by those with a history of entrenched rough sleeping.
- Carry a client case load of 4 – 5 service users and provide face to face support in a variety of settings including their homes, the full range of community locations and EACH’s Harrow based offices.
- Addressing issues of social isolation and exclusion; facilitating service user involvement in the community.
- Providing key work, risk assessment and support planning for service users to enable successful, recovery focused achievement of their personal goals.
- Closely collaborating with LB Harrow Rough Sleeping Co-ordinator and Outreach Team to facilitate referral into the service and participate joint outreach engagement
- Experience of outreach work amongst vulnerable people and providing housing support.
- Experience of supporting people with alcohol and drug problems in a keyworking role.
- Experience of developing and implementing support plans.
- Experience of working with ex-offenders and people with complex needs.
- Experience and knowledge of working within a harm reduction framework
