As a Hospital Social Worker, you'll work at the frontline of care planning and discharge, making a real difference in the lives of vulnerable adults. With your skills and expertise, you'll support timely and safe discharges, helping individuals regain independence and reduce their risk of harm.
You'll:
  • Carry out person-centred assessments in a hospital setting.
  • Use your professional judgment to create effective support and care plans.
  • Work collaboratively with NHS colleagues, families, and other professionals.
  • Ensure all interventions uphold dignity, empowerment, and best practice.
  • Make informed decisions that lead to safe, sustainable outcomes.

✅ Social Work England registered
✅ Minimum of 2-4 years' experience in Adult Social Care
✅ Strong understanding of the Care Act, Mental Capacity Act & safeguarding frameworks
✅ Excellent communication and relationship-building skills
✅ Ability to manage a diverse caseload and prioritise effectively
✅ Commitment to best practice, dignity, and empowermentDesirable Experience
  • Hospital discharge planning or reablement
  • Multi-disciplinary working across health and care
  • Mentoring or supporting junior staff/students
