- 1st level registered Nurse
- 2 years post registration experience
- Post graduate qualification in specialist field or equivalent experience
- Professional degree or diploma in relevant field
- Maintains a portfolio of CPD in line with regulatory body standards
- v300 holder
- Long term condition experience or willingness to undertake
- Community nursing experience
- Experience of MDT working
- Understand principles and framework of Quality and Safety agenda (Clinical Governance)
- Good organisational and presentational skills
- Understanding of Risk Management Issues
- Ability to travel between multiple sites in fulfilment of job role
- Working experience of complex care management
- Experience of effective multi-disciplinary team working
