We support patients who are end of life across Bradford in the community to provide high quality care to patients at the end of life. Working closely with community nursing teams, Fast Track team and Community Palliative Nurse Specialist, you will support patients to prevent unnecessary admissions to hospital, and support them in their preferred place of care.In addition, you will work alongside other key members of the primary health care team, other health and social care professionals, and carers, todevelop and implement personalised care plans for patients at the end of life.This is a great opportunity to gain experience & one in which you will be supported by your colleagues, Community Team Leader and Operational Lead.If you come and join us, our promise to you is that we will support you with your development needs and aspirations.
