- Conduct swift assessments of individuals and families referred to the Fast Social Work Team, prioritizing cases based on urgency and risk.
- Identify immediate safeguarding concerns, risks, and vulnerabilities, and develop appropriate intervention plans.
- Collaborate with partner agencies, such as police, healthcare providers, and housing services, to ensure a coordinated response.
- Provide immediate crisis intervention and support to individuals experiencing emergencies or urgent social care needs.
- Respond to crisis situations promptly, providing emotional support and practical assistance to individuals and families in distress.
- Develop safety plans, risk management strategies, and crisis intervention techniques to mitigate risks and ensure immediate safety.
- Liaise with mental health services, substance abuse programs, and other specialized agencies to provide holistic support during crises.
- Coordinate and facilitate access to emergency accommodation, if needed, for individuals facing homelessness or immediate danger.
- Work collaboratively with individuals, families, and professionals to develop and implement care plans tailored to their specific needs.
- Advocate for clients' rights, ensuring their voices are heard and their choices and preferences are respected.
- Coordinate and facilitate access to appropriate support services, including housing, healthcare, mental health, and substance abuse treatment.
- Provide information, advice, and guidance to individuals and families regarding available resources and entitlements.
- A degree in Social Work or a relevant social science discipline.
- Registration or eligibility for registration with the appropriate regulatory body for social work.
- Experience working in fast-paced social work environments, such as rapid response teams or crisis intervention teams.
