Job IntroductionLincolnshire County Council has an exciting opportunity for a Social Worker Level 1 position based within the Adult Care Team at Lincoln County Hospital. This role would be suitable for a newly qualified Social Worker requiring their ASYE or a Level 1 candidate looking for more experience in a hospital setting.About the RoleThe on-site role involves undertaking assessments of need both within hospitals and the community settings to ensure our customers are able to live independent lives and for those that need support, the right support package, in the right place, at the right time.The customer is at the centre of the process, our highest continuing aim is to provide the best service possible to service users and their families.The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we support are at the very centre of their own discharge. Working creatively when planning support packages, thinking outside of the box to ensure an individual care plan is in place that empowers the service user to participate.LCC is leading the way on the introduction of a fully integrated Discharge to Assess Hub, to support access to the right services, reducing delays and delivering benefits.Work is undertaken face to face, on site, to continually offer the best service we can to those people who we support.You will work as part of the 7-day Adult Care rota. This equates to approximately 1 in 4 weekends worked, and these are all scheduled 3 months in advance.About The TeamWe are a team of 26 Community Care Officers and Social Workers based within Lincoln County Hospital.
