  • Engage with service users, their families, and carers to identify their needs and support them in achieving personal outcomes, promoting independence.
  • Perform person-centred integrated assessments and develop care plans that are aligned with individual strengths and ‘what matters’ to them.
  • Maintain a limited caseload, ensuring quality service delivery.
  • Provide accurate advice and information, supporting individuals to make informed decisions about care and funding.
  • Participate in joint assessments, particularly in complex healthcare scenarios requiring thorough analysis and reporting.
  • Contribute positively to social work assessments, including those necessary for continuing health care evaluations.
  • Support community-based solutions, collaborating with various partners to deliver quality services.
