
Practice Nurse

- Lead proactive, evidence-based nursing care for people living with long-term conditions, supporting self-management and preventing avoidable deterioration.
- Provide high-quality treatment room and preventative care services across the age range, guided by current best practice and professional standards.
- Offer clear health education and personalised advice, helping patients understand their conditions and treatment plans.
- Work closely with GPs, HCAs, pharmacists and wider MDT colleagues to coordinate safe, joined-up care.
- Use clinical systems to record accurately, monitor outcomes and contribute to quality improvement, safety audits and practice targets.
- Play an active role in safeguarding, infection prevention and maintaining a positive, patient-centred culture at the surgery.
- Registered General Nurse (RGN) with current NMC registration
- Completion of a Practice Nurse Induction Programme or General Practice Nurse (GPN) Foundation Course
- Experience in a general practice or primary care setting
- Competent in chronic disease management, cervical cytology, immunisations, and wound care
- Excellent communication, record-keeping, and time management skills
- Experience using SystmOne
- Additional training in travel health, smoking cessation, contraception, or spirometry
- Awareness of QOF and its relevance to nursing care
- Compassionate, patient-focused approach
- Ability to work autonomously and as part of a multidisciplinary team
- Committed to ongoing professional development
