  • Lead proactive, evidence-based nursing care for people living with long-term conditions, supporting self-management and preventing avoidable deterioration.
  • Provide high-quality treatment room and preventative care services across the age range, guided by current best practice and professional standards.
  • Offer clear health education and personalised advice, helping patients understand their conditions and treatment plans.
  • Work closely with GPs, HCAs, pharmacists and wider MDT colleagues to coordinate safe, joined-up care.
  • Use clinical systems to record accurately, monitor outcomes and contribute to quality improvement, safety audits and practice targets.
  • Play an active role in safeguarding, infection prevention and maintaining a positive, patient-centred culture at the surgery.

  • Completion of a Practice Nurse Induction Programme or General Practice Nurse (GPN) Foundation Course
  • Experience in a general practice or primary care setting

  • Excellent communication, record-keeping, and time management skills
Desirable
  • Experience using SystmOne
  • Additional training in travel health, smoking cessation, contraception, or spirometry
  • Awareness of QOF and its relevance to nursing care
