- Knowledge of and experience of working with patients with long term conditions
- Able to request and interpret investigations and results
- Able to work and make decisions autonomously, working within own professional boundaries
- Clinical leadership skills
- Minimum of 3 years post registration experience
- Evidence of CPD
- Able to evaluate the safety and effectiveness of own practise
- Understanding of NHS strategy, particularly related to primary care
- Knowledge of primary and community healthcare
- Clinical audit knowledge and skills
- Knowledge and experience of safeguarding
- Experience of working with SystmOne
- Commitment to reducing health inequalities and proactively working to reach people from all communities
- Demonstrates use of appropriate communication to gain the cooperation of relevant stakeholders
- Is able to recognise personal limitations and refer to other colleagues/agencies when necessary
- Ability to identify and manage risk when working with individuals
- Ability to work under pressure and meet deadlines
