We are looking for an experienced nurse to join our PCN in an exciting new role that has been developed to work alongside our multi disciplinary team, and also work closely with the PCN's Practices and their teams to provide care and treatment to patients presenting in primary care and community care settings, including their own homes and care homes.Main duties of the jobThe postholder will work autonomously and within their professional boundary to deliver planned ambulatory care services to the PCN's patient population, focussing on patients who are hard to engage, taking an holistic view and adressing a wide variety of needs.The postholder will provide expert advice to patients, carers and colleagues to ensure the maintenance of clinical excellence and provide advice in respect of ongoing care needs, being accountable for their own professional actions within professional guidelines.The postholder will be supported by the PCN's Clinical Director and work collaboratively within the multi disciplinary team to meet the needs of patients, support the delivery of policy and procedures and improve our service delivery.About usLeicester City South Primary Care Network provides care to the 32,000 patients that make up the practices of Saffron Health, The Hedges Medical Centre, Sturdee Road, Inclusion Health Care and Leicester City Assist.
