Job Category: EducationJob Description:Education Health Care Plan Coordinator | 8860 | Secondment Opportunity / Temporary Contract for 18 months | 29.6 hours per week | £39,862 to £41,771 pro rata per annum | Grade J | NorfolkAre you an enthusiastic professional, highly skilled in relationship-based practice?If you have experience in the special education sector and a commitment to facilitating positive outcomes for children and young people, we want to hear from you.We have an opportunity to join our highly skilled team within the Family Help & High Needs SEND Service, with a focus on driving forward performance improvement and quality of Education Health Care Plans (EHCPs).As an EHCP Coordinator, you will play a crucial role in navigating the EHCP process, collaborating with stakeholders, and ensuring comprehensive support for individuals.Your responsibilities will include:
  • Collaborating with schools, parents, healthcare professionals, and local authority colleagues to gather relevant information
  • Coordinating and managing the EHCP assessment, review and placement planning process
  • Ensuring that EHCPs are accurate, comprehensive, and reflective of the individual's needs
  • Liaising with external stakeholders to ensure the seamless delivery of support services
  • Keeping up to date with legislative changes and best practices in the field
