- Ensure practice is in accordance with the NMC code of Professional conduct and work within limits of competence
- Ensure the integration of individualised health plans, hospital passports and admission plans into the practice of healthcare professionals within the acute hospital setting
- Provide specialist advice and support to hospital staff, ensuring there is a clear pathway of care for people with learning disabilities and / or autism
- Prioritise and respond to referrals made to the service
- Respond to unplanned/emergency admissions where a need for support/advice is identified
- Ensure practice is in accordance with the NMC code of Professional conduct and work within limits of competence

- Extensive experience of working directly with people with learning disabilities and Autistic people in a variety of health care settings
- Good understanding of acute and primary care services and health care issues, including health inequality and barriers to equitable access
- Significant experience of working in an acute care setting
- Excellent communication and inter-personal skills, including alternative and augmentative communication skills
- Ability to work independently, prioritise and organise own workload to meet the needs of patients and the service
